DESCRIPTION:Dans le cadre des permanences de médiation santé au sein du Centre Municipal de Santé de Soyaux\, la Médiatrice Santé d’OMEGA\, accompagne les patients dans leurs démarches de santé et dans l’utilisation des outils numérique s: Mon Espace Santé\, AMELI\, Doctolib… \nSes missions principales sont :\n– Aider à l’activation et à l’utilisation de Mon Espace Santé\n– Accompagner à la prise de rendez-vous médicaux\, notamment auprès de médecins spécialistes\n– Informer et sensibiliser aux dépistages organisés (cancer du sein\, du col de l’utérus et colorectal) \nCes permanences ont pour but de faciliter l’accès aux droits et aux soins\, d’aider les patients à être plus autonomes et de faire de la prévention en santé.
